I assume responsibility for all charges for the care of my animals. I also understand that these charges will be paid at the time of release. I understand and accept that a late charge of $15.00 per month will be added to any unpaid invoice, past 30 days, and if my account is turned over to collections, all costs will be added to the outstanding balance. I understand that if I schedule an appointment and cannot make it and do not contact Quality Pet Care 24-hours in advance to cancel, I will be required to prepay a non-refundable deposit at the time any future appointment is scheduled.

If you want your pet’s records sent to another facility (groomer, boarding facility, another veterinary hospital, etc.) we need your authorization to do so. Please fill out the bottom of this page to complete this. You may list additional pets as well.
